About This Notice

Wylde Self is a patient engagement and behavior change platform that facilitates care coordination between patients and their existing healthcare providers. When you connect with a clinician through the Platform, certain data you provide may be considered protected health information (PHI) under HIPAA.

This notice explains our practices regarding the use and disclosure of your PHI, and your rights concerning that information.

How We Use and Disclose Your PHI

Treatment Support

We may use your PHI to support the care your linked clinician provides to you. This includes:

Care Coordination

With your consent, we share relevant health data with your linked clinician to support coordinated care. This may include sharing adherence data, check-in summaries, and protocol status to help your clinician make informed decisions about your care plan.

Disclosures with Your Consent

Your PHI is shared with your clinician only when you have explicitly initiated the connection by generating or entering a share code. You control this relationship and can revoke access at any time.

Required by Law

We may disclose your PHI when required to do so by federal, state, or local law. This includes disclosures to government authorities for public health activities, health oversight, judicial proceedings, or law enforcement purposes as required by applicable law.

Your Rights

Right to Access

You have the right to inspect and obtain a copy of your PHI maintained by Wylde Self. You can access your health data directly through the app at any time. For formal access requests, contact our Privacy Officer.

Right to Amendment

You have the right to request that we amend your PHI if you believe it is incorrect or incomplete. You can update most data directly in the app. For clinical data managed by your provider, amendments may need to be coordinated with your clinician.

Right to Restriction

You have the right to request restrictions on certain uses and disclosures of your PHI. While we will consider all restriction requests, we are not required to agree to a restriction except as required by law. You can restrict clinician access at any time by revoking your care team connection.

Right to Accounting of Disclosures

You have the right to request a list of certain disclosures of your PHI that we have made. This does not include disclosures made for treatment, payment, or healthcare operations, or disclosures you authorized. Contact our Privacy Officer for an accounting request.

Right to Confidential Communications

You have the right to request that we communicate with you about health matters in a particular way or at a certain location. We will accommodate reasonable requests.

Right to a Copy of This Notice

You have the right to a paper or electronic copy of this notice at any time. This notice is always available at wyldeself.com/hipaa.

Our Responsibilities

Wylde Self is committed to maintaining the privacy and security of your health information. Our responsibilities include:

  • Maintaining appropriate administrative, technical, and physical safeguards to protect your PHI
  • Following the terms of this notice as currently in effect
  • Notifying you if we are unable to agree to a requested restriction on use or disclosure
  • Entering into Business Associate Agreements with service providers who handle PHI on our behalf
  • Using or disclosing only the minimum necessary amount of PHI to accomplish the intended purpose
